What's required to play?

All Sultan Athletes need a "Blue Card" to practice (what athletes receive when all paperwork is turned in):

  • Middle School athletes do not need to pass the AIA "Brainbook" and Opioid classes.

  • High School athletes need to have all the boxes checked off.

Yearly Sports Physical

  • YRMC PhysicianCare of Bagdad provides sports physicals for $25. Please call, 633-6011 to schedule. The Clinic has the form or you may print it here.

MTBI AIA Concussion Form

Proof of Health Insurance Policy

  • snap a photo and email it to or stop by with it in hand and we will copy it

  • If you do not have health insurance, we have information for an affordable student policy specific to playing sports

"Brainbook" and "Opioid Education"

  • Students can login to the AIA Academy Prep365 Site and complete the course. (MS athletes DO NOT NEED TO DO THIS). This needs to be taken and passed only ONCE during their High School Career. If students completed this course at another high school, they must fill out the transfer form, found here on AIA's website . There's no need to print the certificate- we are notified once it is passed!

Pay to Play

  • Pay to Play must be paid before the first game.
